Manatee PHO Contract Summary and Customary Plan Effective Dates
Interplan Health Group D/B/A Accountable Health Plan – Credentialing is delegated to the PHO. The effective date for new physicians is the PHO notification effective date.
Beech Street – Approximately 30 business days following receipt of the PHO memo.
Evolutions – Credentialing is delegated to the PHO. The effective date for new physicians is the PHO notification effective date.
Freedom Health, Inc. – Credentialing is delegated to the PHO. The effective date for new physicians is the PHO notification effective date.
Gulf Coast Provider Network (Formerly Healthcare Sarasota) – If the update is received by the 15th of the month, the effective date will be the 1st of that month. If the update is received after the 15th of the month, the effective date will be the 1st of the following month.
Humana – The PHO effective date if already a provider with Humana. If you are not a provider, a credentialing period of approximately two months.
Manasota Health Preferred Network – The PHO effective date.
MCM Max Care –The PHO effective date.
MultiPlan/PHCS – Credentialing is delegated to the PHO. The effective date for new physicians is the PHO notification effective date.
Senior Care Plus – The PHO effective date.
